All of the HR forms must be completed before the end of the month. Please get them in before Thanksgiving. If you do not have them filled out, you will lose benefits. Make sure that you obtain the BCBS Dental form. We are switching Dental programs and will lose your dental coverage if you do not fill it out and turn in with the rest. The 401K info should go out with upcoming the pay stubs.
The shoe van will be coming on Friday instead of Thursday for this week.
